    1. Details of the eye test results, which can be uploaded for a file or attached as a picture. If you prefer you can simply type the prescription powers into the text box provided.
    2. We require a PD measurement, which is the distance between pupils. For a more detailed explanation follow this LINK
    3. Please note prescription lenses are not cancellable or refundable as stated in our terms and conditions.
